Key Responsibilities
  • Compliance and Quality Assurance: Ensure compliance with corporate and departmental policies, procedures, and quality assurance methods. Demonstrate adherence to licensing, regulatory, and accrediting agency requirements.
  • Communication and Teamwork: Maintain professional communication with patients, customers, and colleagues. Foster a collaborative environment by working effectively with management and peers.
  • Medication Management: Monitor and manage medication therapy regimens, identifying potential contraindications, interactions, and duplications. Address patient medication requests and coordinate alternative therapy with clinical pharmacy specialists.
  • Compounding and Dispensing: Compound and dispense pharmaceuticals, including sterile, chemotherapy, and oral preparations. Ensure accuracy and quality control.
  • Controlled Substances and Records: Issue controlled substances to patient care areas and maintain required records.
  • Pharmaceutical Care Planning: Implement, evaluate, and redesign pharmaceutical care plans based on patient progress and evolving goals.
  • Clinical Services: Provide clinical services, including vancomycin dosing/monitoring, patient medication counseling, antibiotic stewardship, renal function monitoring, IV to PO conversion, discharge medication reconciliation, and admission medication reconciliation.
  • Formulary Management: Sustain the Princetons formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
