Job Description: Healthcare Support Specialist

We are looking for a dedicated and empathetic individual to take on the role of Healthcare Support Specialist at Clinical Professional Connections. This position involves providing essential assistance to members in managing their healthcare needs, prioritizing their safety, wellness, and overall health.

Key Responsibilities:

  • Organize and manage healthcare appointments for members, encompassing medical, dental, and specialist consultations.
  • Promote healthy eating habits and dietary compliance for members, ensuring adherence to recommended nutritional guidelines.
  • Offer emotional support and a compassionate ear for members, providing comfort and understanding during difficult times.
  • Collaborate with healthcare professionals to facilitate effective communication and care coordination for members.
  • Evaluate and monitor the overall health status of members, reporting any issues or changes to the relevant healthcare providers.
  • Act as a bridge between members and healthcare professionals, ensuring clear communication and comprehension of medical instructions.
  • Keep precise and current records of members' healthcare requirements, appointments, and progress.

Qualifications:

  • Valid Licensed Practical Nurse (LPN) license; or Certified Nursing Assistant (CNA) or Medical Assistant (MA) license in Texas.
  • A minimum of [3] years of experience in a healthcare coordination or support capacity.
  • Exceptional communication and interpersonal abilities, with a talent for listening and providing emotional support.
  • Strong organizational skills and meticulous attention to detail, capable of efficiently scheduling appointments and maintaining accurate records.
  • A humble and approachable demeanor, with a sincere commitment to assisting and supporting members.
  • Ability to work independently while effectively collaborating with healthcare professionals.
  • Willingness to work onsite at least two days a week for [6] hours total (3 hours per day/2 days), with potential for this role to evolve into a part-time or full-time position.
