About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at The Riverton Rehabilitation and Healthcare Center. As a LPN, you will play a vital role in providing high-quality patient care in a sub-acute and long-term care setting.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Collaborate with the healthcare team to develop and implement individualized care plans
  • Conduct thorough assessments and document patient progress
  • Participate in interdisciplinary rounds and contribute to the development of patient care plans
Requirements
  • A current, unencumbered LPN license in this state
  • Experience in a sub-acute and/or long-term care setting
  • Strong communication and interpersonal skills
