Hospice Medical Social Worker Job Description

We are seeking a compassionate and skilled Medical Social Worker to join our team at Heart n Home Hospice of Baker City, a part of LHC Group. As a Medical Social Worker, you will play a vital role in providing medical social services under the direction of a physician and Interdisciplinary Group to assist in the understanding of significant social and emotional factors related to the patient's health status and in development of coping mechanisms.

Responsibilities

The Medical Social Worker will assist the patient, significant others, physician, and health care team staff to understand significant personal, emotional, environmental, and social factors related to the patient's health status as needed. They will contribute as a health care team member to the development of a comprehensive, integrated Plan of Care for patients daily. Additionally, they will instruct health care team members on community resources available to assist patients as needed and establish a Plan of care that addresses bereavement needs with clear delineation of services to be provided and frequency of service delivery up to thirteen (13) months following the death of the patient.

Qualifications

The ideal candidate will have a Bachelor's Degree in social work from a school accredited by the Council on Social Work Education or Bachelor's Degree in psychology, sociology, or other field related to social work and supervised by MSW. They will also have one year of social work experience in a health care setting and current CPR certification. Additionally, they must be licensed as a Social Worker from the State Board of Social Work in Oregon.
