The Salvation Army USA Central Territory is seeking a highly skilled and compassionate Lead Grant Per Diem Case Manager to join our team. As a key member of our Transitional Housing Grant Per Diem program, you will be responsible for leading, overseeing, and coordinating the activities of veterans residing in our facilities.
Key Responsibilities:- Professionally complete case management functions, including intakes, assessments, service plans, progress notes, and incident reports.
- Guide the case management team and provide support for productive day-to-day operations.
- Manage an organized bookkeeping system and maintain confidentiality and professional practices.
- Organize weekly classes and groups, incorporating life skills, employment skills, and community resources for clients.
- Complete and provide regular reports and documentation, including daily, weekly, monthly, quarterly, and annual activities and progress measurements.
- Coordinate outreach, admission, and discharge with the Health Care for Homeless Veteran (HCHV) Grant Per Diem Liaison and staff.
- Manage census and client accountability according to operating protocol.
- Oversee case management engagement and follow up with case coordination to facilitate client goal achievement.
- Utilize community partnerships and resources to enhance support for clients.
- Participate in weekly staff meetings and supervision.
- Provide updates and insights to Program Directors regarding client progress and resources needed.
- Maintain updated client files and utilize recommended approaches and modules to support program goals.
- Learn and maintain HMIS data information and other programming software.
- Manage oversight of client daily living, including home living routine, cleaning, saving plan, and medication monitoring.
- Review policies and procedures regularly and support the mission of The Salvation Army and Veteran Residence program.
- Bachelor's degree in human service or social service field or related area, plus one year of work experience.
- Experience working with recovering substance abusers and veterans is a plus.
- Combination of education and work experience will be considered.
- Certified Alcohol and Drug Counselor (CADC) or Recognized Associate Substance Abuse Counselor (RASAC) or LPC or LCSW is a plus.
- Excellent interpersonal and communication skills, understanding of the homeless and chemically dependent veteran population.
- Ability to think independently and exercise sound judgment.
