Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Beaver Meadows, Pennsylvania.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in the comfort of their own homes. As a Home Health Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote independence, safety, and overall well-being.

Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Develop and implement manual therapeutic exercises, gait training, and other interventions to improve clients' physical function.
  • Collaborate with nursing staff to revise care plans and ensure seamless care coordination.
  • Educate clients and their families on rehabilitative care and activities to promote independence and safety.
Qualifications:
  • Current PAPhysical Therapist - PT license.
  • Minimum of one year of recent work experience as a Physical Therapist - PT.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
