We are seeking a highly skilled Case Management Professional to join our team at Denton County MHMR. In this role, you will work closely with clients, families, and service providers to develop and implement individualized plans.

Key Responsibilities
  • Develop and maintain relationships with community resources and service providers to ensure access to necessary services.
  • Collaborate with clients and their families to establish and achieve goals, providing ongoing support and guidance.
  • Ensure accurate and timely documentation of services provided, including completion of required paperwork.
  • Provide emergency/crisis intervention when necessary, adhering to HHSC and Center policies and procedures.
Requirements
  • Possession of a Master's Degree in a human services field or equivalent experience.
  • Previous crisis experience preferred, with English/Spanish bilingualism also desirable.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Familiarity with Texas Administrative Code (TAC) and fidelity tools, with ability to maintain up-to-date knowledge and skills.
