Medical Director Physician Engagement and CDQI – Manhattan, NY

About the Role
Mount Sinai Medical Center is seeking a highly skilled Medical Director to lead our Physician Engagement and Population Health initiatives. As a key member of our team, you will be responsible for driving population health goals, including access, quality, utilization, and accurate risk adjustment.
Key Responsibilities
- Develop and implement population health strategies to improve patient outcomes and reduce healthcare costs.
- Collaborate with physicians, medical directors, and other key stakeholders to drive value-based care success.
- Establish and maintain productive working relationships with practice managers, directors, and other practice administrative staff.
- Lead the Population Health Practice Optimization team to improve the efficiency of care, quality, and accurate risk adjustment for all populations served.
- Present and facilitate pod and practice health meetings to drive value-based care success.
- Help administer the Value-Based care bonus program for PCPs (IM, FP, and Geriatrics).
- Respond to provider/administrator/practice requests from meetings and ad-hoc communications.
- Stay clinically active in primary care or another setting.
Requirements
- MD or DO degree, Internal medicine, Family medicine, or Geriatrics strongly preferred, board certification required.
- Value-based care knowledge and experience.
- Demonstrated experience in successfully driving population health initiatives.
- Track record and experience in implementing CDQI initiatives and successfully engaging physicians and practice.
- Track record of a skillful presenter and influencer.
- Demonstrated ability to see the forest, leaves, and trees at the same time and move initiatives forward.
- Accomplishment in rapidly establishing clinical, population health, and operational credibility with key practice stakeholders.
- Experience and passion for primary care excellence.

- Experience in clinical transformation: developing and implementing quality programs, care pathways, clinical guidelines, and best clinical and operational practices in primary care.
Compensation and Benefits
Salary range from $230,000 to $260,000 (when scaled to 1 FTE) (not including bonuses/incentive compensation or benefits).
