Job Title: Hospitalist Advanced Practice Provider

DispatchHealth is seeking a skilled Hospitalist Advanced Practice Provider to join our team. As a key member of our care delivery team, you will provide high-quality, patient-centered care to individuals in their homes.

Key Responsibilities:
  • Provide direct, hospital-level care to patients in their homes, including performing clinical assessments, developing and ordering hospital-level and post-acute level internal medicine clinical workups, and documenting patient information.
  • Collaborate with the Advanced Care clinical team to create and implement multidisciplinary plans of care for patients on Advanced Care, Extended Care, and Bridge Care service lines.
  • Participate in program enhancement for Advanced Care, Extended Care, and Bridge Care service lines, and continuously enhance the Advanced Care commitment to quality, safety, and experience.
  • Work closely with DispatchHealth Advanced Care and Acute Care APPs to foster strong and longitudinal relationships, and participate in education and engagement around Advanced Care program services.
Requirements:
  • Graduate with an advanced degree from an accredited institution.
  • 2-3 years of hospitalist or hospitalist equivalent experience.
  • Current unrestricted state license as a Nurse Practitioner or Physician Associate.
  • Nationally board certified, current.
  • Current BLS / ACLS certification.
  • Prescriptive authority and DEA.
  • Ability to lift and carry equipment up to 50 pounds.
  • Ability to work a varied schedule with evenings, holidays, and weekends required.
