Role Overview:

As a Comprehensive Case Management Specialist, you will engage in providing thorough case management services. This role is essential in delivering environmental support and care coordination to enhance individual functioning, facilitate access to vital services, and foster a recovery-oriented environment as outlined in the Individual Recovery Plan (IRP).

Compensation: The anticipated pay range for this position is between $15 and $18 per hour, commensurate with experience. Additionally, a hiring bonus of $1,000 is available.


Key Responsibilities:

The Comprehensive Case Management Specialist will successfully intervene by assisting individuals with:

  • Establishing natural supports to encourage community integration.
  • Identifying and addressing service needs.
  • Linking individuals to necessary services and resources as identified in the service planning process.
  • Coordinating services outlined in the IRP to enhance service integration and reduce service gaps.
  • Ensuring the IRP remains adequate to meet evolving needs.

Essential Functions:

Engagement and Needs Assessment:
The case manager will foster a recovery-based partnership that encourages personal accountability, providing support, hope, and motivation. This includes helping individuals develop a community support network to promote integration and housing stability, as well as collaborating to identify and prioritize housing, service, and resource needs for the IRP.

Care Coordination:
The case manager will oversee care activities, assisting individuals as they navigate various services and supports. This involves effective communication among the individual, their primary and specialty providers, and other identified supports to ensure comprehensive service delivery.

Referral and Linkage:
The case manager will facilitate referrals and connections to services and resources identified in the IRP, including housing, social supports, and entitlements. This may involve assisting individuals in locating resources, scheduling appointments, completing application processes, and arranging transportation as necessary.

Monitoring and Follow-Up:
The case manager will conduct community visits to assess progress towards IRP goals and gather feedback on satisfaction with treatment. Ongoing monitoring will ensure that services align with the IRP and effectively address individual needs.


Qualifications:
  • Must meet paraprofessional status requirements as per relevant guidelines.
  • Effective oral and written communication skills are essential.
  • Understanding and sensitivity to diverse populations are required.
  • A Bachelor's degree in Psychology, Social Work, or a related field is necessary.
  • A valid driver's license with a clean record is required.
