Job Title: Care Navigator

Acentra Health is seeking a skilled Care Navigator to join our team. As a Care Navigator, you will play a vital role in empowering better health outcomes through technology, services, and clinical expertise.

Job Summary:

The Care Navigator will assist members in accessing the right care at the right time by building strong relationships that support member engagement in their medical and/or behavioral health care treatment plans. This role requires a commitment to removing barriers to care by identifying needed resources, helping members navigate through healthcare services and systems, promoting holistic health through education, and reducing social barriers.

Key Responsibilities:
  • Outreach to members to assist them in meeting their preventative health needs.
  • Complete health and social needs assessments to assist in the development of individualized care plans.
  • Work within a patient's benefit network for the best outcome and to enhance access to care.
  • Potentially cross-train to support other contracts within Acentra Health to provide a flexible workforce and meet client/customer needs.
Requirements:
  • Bachelor's degree in a health-related field (e.g., Social Work, Counseling, Psychology, or another closely related healthcare field) with 1+ years of experience in medical, mental health, and/or behavioral healthcare, social work, community outreach, or patient advocacy.
  • 2+ years of experience in medical, physical health, mental health, and/or behavioral healthcare, social work, community outreach, or patient advocacy in a clinical field, remote, or onsite environment.
  • Understanding or experience in completing health or social assessments.
  • Ability to demonstrate purposeful and outcomes-directed critical thinking within daily operational activities.
  • Must be comfortable educating members and developing an individualized plan of care.
  • Must be comfortable working telephonically and with computer systems while assisting members.
  • Must be able and comfortable with maintaining metrics and goals.
  • Must be comfortable providing outreach to members.
  • Excellent verbal/written communication skills.
  • Ability to work in a team environment.
  • Flexibility and strong organizational skills.
  • Must be proficient in MS Office and Internet/Web navigation.
