About the Role

We are seeking a highly skilled and compassionate Care Planner to join our team at SCAN Health Plan. As a Care Planner, you will play a critical role in providing long-term care management to vulnerable older adults, disabled adults, and caregivers, helping them remain safely at home and preventing premature institutionalization.

Key Responsibilities
  • Conduct in-home and telephonic assessments, care planning, and client monitoring through contacts with the client, professionals, and client's support system.
  • Coordinate linkages with community-based services and resources.
  • Maintain accurate and timely documentation of client cases, including assessments, care plans, service arrangements, and client monitoring progress notes.
  • Ensure timely implementation of services for clients by completing all work within required timeframes.
  • Develop and implement comprehensive care plans tailored to client objectives and goals, with measurable timeframes and interventions outlined.
  • Maintain effective communication with other disciplines working with clients, provide feedback on psychosocial aspects of client situations, and offer informal training to fellow care planners and staff.
  • Maintain accurate purchase of service records by completing timely monthly service authorizations and verifications to contracted vendors, following contract guidelines for service expenditures.
  • Stay current on Medicare and Medi-Cal policies, Federal and State legislation affecting health and social services, and maintain updated community resource information.
  • Participate in ongoing outreach activities to maintain census and provide excellent customer service in the community.
  • Regularly participate in Case Conference, Quality Assurance chart reviews, and other audits as required, helping to train new employees and responding positively to suggestions for improvement.
Requirements
  • Bachelor's Degree in Social Work, Psychology, Gerontology, or related field required; Master's Degree in Social Work (MSW) preferred.
  • BILINGUAL: Must be bilingual in English/Spanish, with proficiency to be assessed through a test.
  • 1-2 years of experience working with the elderly.
  • 1+ year of experience working in a hospital, home health agency, or related setting with a primary targeted population of older adults.
  • Ability to travel within the program's catchment area and visit client homes, with a valid driver's license, automobile insurance, and reliable transportation.
  • Strong organizational, verbal, and written communication skills, with a demonstrated customer focus and ability to work well with others.
  • Experience working in a case management setting or demonstrated knowledge of case management techniques preferred.
