Community Engagement Specialist: Bronx Area

POSITION SUMMARY: Under the guidance of the Project Director, the Community Engagement Specialist will connect with clients in the community through a trauma-informed approach. This full-time role involves training in all five Evidence-Based Treatments (EBTs) outlined for this initiative when interacting with clients.

The Specialist will offer peer support services, which include but are not limited to screening, referrals, and linking clients to appropriate care. They will also provide educational resources and distribute materials focused on prevention and harm reduction.

Additionally, the Specialist will foster relationships with trusted community partners and gather GPRA data. Utilizing Electronic Health Records (EHR), they will document client interactions and monitor referral outcomes.

KEY RESPONSIBILITIES:

  1. Encourage clients with substance use challenges to pursue and maintain care.
  2. Implement evidence-based practices to engage and deliver services through a trauma-informed lens.
  3. Collect GPRA data accurately.
  4. Provide social support and recovery services, including referrals for Medication-Assisted Treatment (MAT) and other health concerns.
  5. Facilitate connections to prevention and harm reduction services.
  6. Educate clients about life-saving interventions, such as overdose reversal medications and distribute relevant resources.
  7. Train clients and community members on the use of opioid overdose reversal medications, including naloxone.
  8. Develop partnerships with community organizations.
  9. Document client interactions and track referral outcomes using EHR.
  10. Engage in outreach initiatives to represent the organization.
  11. Comply with confidentiality policies and HIPAA regulations.
  12. Participate in job training, educational programs, meetings, and committees as assigned, including the Advisory Council.

QUALIFICATIONS:

  • High School Diploma or GED.
  • Personal experience with substance use disorders.
  • Previous experience working with the target population.
  • Proficient in reading, writing, and speaking English.
  • Familiarity with computer applications.
  • Strong organizational skills.
  • Bilingual in Spanish is required.
