About the Role

The Bronx Community Health Network is seeking a highly skilled Family Medicine Physician to join our team at our Health Center in White Plains Road, Bronx. As a Family Medicine Physician, you will be responsible for providing comprehensive primary care services to patients of all ages, conducting medical examinations and assessments, and prescribing medications and treatments.

Key Responsibilities
  • Provide high-quality patient care and set the clinical standard for delivery of optimal patient services
  • Conduct comprehensive medical examinations and assessments of patients
  • Elicit and record information regarding patient's medical history electronically
  • Order or execute various tests, analyses, and diagnostic images to provide information on patient's condition
  • Prescribe medications and other appropriate treatments
  • Provide patients with health education about preventive care measures, chronic disease self-management, and healthy lifestyle choices
  • Engage in disciplinary care team meetings to discuss patient treatment plans
  • Collaborate with other healthcare professionals to coordinate patient care
  • Supervise the mid-level practitioners' review and co-sign charts for Nurse Practitioners, and other clinical staff as necessary
  • Referring patients to specialists as needed for further evaluation and treatment
  • Contribute to the development of policies and standards of patient care by participating in committees
Requirements
  • Licensed in the State of New York State
  • Board Certified/Board Eligible in Internal Medicine or Family Medicine
  • Graduation from an Accredited Medical School and completed Accredited Residency Program
  • 2 to 5 years of experience post board certification
  • Clinical and administrative experience in a family or community health center, or ambulatory care center
  • Experience with Athena Health EMR (preferred)
What We Offer
