We are seeking a skilled Physician Rehabilitation Specialist to join our team at JAG Physical Therapy. As a key member of our multidisciplinary team, you will play a vital role in delivering exceptional patient care and contributing to our mission of providing rehabilitative excellence.
About the RoleThis is an exciting opportunity for a motivated and compassionate individual to make a real difference in the lives of our patients. As a Physician Rehabilitation Specialist, you will be responsible for assessing patients' needs, developing personalized treatment plans, and collaborating with other healthcare professionals to ensure optimal outcomes.
Key Responsibilities- Assess patients' physical function and develop targeted treatment plans to address their unique needs and goals
- Conduct thorough evaluations, including initial assessments, daily notes, reassessments, progress reports, and discharge planning
- Develop and implement effective treatment plans, incorporating evidence-based practices and staying up-to-date with industry developments
- Communicate effectively with patients, families, and interdisciplinary teams to ensure seamless care coordination
- Maintain accurate and timely documentation, adhering to regulatory requirements and departmental standards
To be successful in this role, you will require:
- A Bachelor's or Master's degree in a relevant field, preferably with a Doctorate degree
- Licensure as a Physical Therapist in the state where you will be practicing
- Completed CEU requirements in your state (NY, NJ)
- Valid CPR certification
- Experience with EMR software, preferred
- Excellent communication, clinical decision-making, and interpersonal skills
