Position Overview
The role of the Care Manager RN involves a collaborative approach to case management, integrating the efforts of patients, families, nursing staff, social workers, physicians, insurance providers, and community resources. This process is essential for ensuring effective communication and seamless care transitions throughout the healthcare continuum.
At Providence, we recognize the importance of patient autonomy, the impact of social determinants on health, and the intricate challenges that may affect patient care, including barriers to coping, mental health issues, substance use, and resource limitations.
Essential Qualifications:
- Bachelor’s Degree in a relevant field
- Active California Registered Nurse License upon hire
- Minimum of 2 years’ experience in a healthcare setting (Acute, Ambulatory, or Post-Acute)
- At least 2 years’ experience in Acute Care Management (Care Coordination or Utilization Management) or completion of the Transitions in Practice (TIP) program for Care Managers
Preferred Qualifications:
- Master’s Degree in a health-related or scientific discipline
