Job Summary:
We are seeking a skilled Hospice Care Coordinator to join our team. This role requires a strong understanding of hospice care and the ability to communicate effectively with patients, families, and healthcare professionals.

Key Responsibilities:
* Facilitate the admission process to and from hospital and long-term care-based referral sources.
* Assess referred patient's clinical needs and disease status and confers with patient's attending physician and hospice physician regarding assessment to establish eligibility.
* Provide education to hospitals and facilities on the purpose, process, and goal of general inpatient care.

Qualifications:
* RN License is required, Bachelor's degree in Nursing highly preferred.
