Job Title: Interim Medical Coordinator

Description

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are seeking an Interim Medical Coordinator in a dynamic environment where you will play a crucial role in ensuring the health and well-being of our clients.

Responsibilities
  • Schedule and coordinate medical appointments for clients, ensuring compliance with state licensing regulations.
  • Monitor client status through regular follow-ups and documentation.
  • Track and record client medical information, including annual and emergency appointments.
  • Communicate effectively with medical professionals, program specialists, supervisors, and directors to ensure seamless care coordination.
  • Provide accurate and timely documentation of client medical appointments.
  • Attend medical appointments as needed to ensure the best possible outcomes for clients.
  • Collaborate with colleagues and supervisors to address client needs and concerns.
  • Perform additional tasks and responsibilities as assigned.
Requirements
  • High School Diploma or GED; Bachelor's degree preferred.
  • Valid driver's license with a satisfactory driving record.
  • Completion of state and agency required training.
  • Ability to establish strong relationships with clients and colleagues.
  • Excellent communication and documentation skills.
