Job Summary

The Overlook is seeking a highly skilled and experienced Clinical Practice Manager to lead our clinical services and operations. As a key member of our team, you will be responsible for directing and supervising both clinical services and office operations.

Key Responsibilities
  • Direct and supervise clinical services and office operations
  • Guide the team in planning and providing individualized care and comprehensive services to patients
  • Provide education in group and individual settings for clinical staff
  • Lead case conference meetings and participate in meetings as assigned
  • Participate in QAPI reviews and provide input on clinical and financial key performance measures
  • Monitor and complete assigned workflow tasks in HCHB
  • Provide clinical direction to field staff as needed
  • Code commercial OASIS
  • Organize educational programs to address identified needs
  • Review clinical records for completeness
  • Demonstrate prioritization, organization, and appropriate management of time
  • Keep current with all regulatory requirements affecting Home Health services
  • Oversee Medical Supply orders
  • Collaborate with supervisory staff to promote accuracy and effectiveness in documentation
  • Participate and lead case conferences and staff meetings
  • Educate staff on deficiencies found through audits as needed
  • Evaluate clinical competency of nursing staff
  • Participate in the orientation of new team members
  • Manage Telehealth programs
  • Perform Blood Pressure clinics as requested
  • Assist with clinical oversight at the Overlook IL and AFL as needed
  • Provide direct skilled nursing care as per physician orders in the field with patient visits as needed
  • On-Call triage coverage as needed
  • Document medical records and prepare reports as needed
  • Educate patients and families in self-care and health management
  • Collaborate with the physician and other home health care providers to enhance patient care
  • Adhere to policies and procedures
  • Participate in agency quality management and safety activities
  • Serve as coverage role during staffing challenges
Requirements
  • Graduate of an accredited school of nursing or physical therapy required
  • Minimum 3 years' experience in a managerial role
  • Minimum 5 years' experience in a patient care setting
  • Current license to practice as a Registered Nurse/Physical Therapist in the state of Massachusetts required
  • Current CPR required
  • Current MA Driver's License and reliable transportation required
  • Demonstrated clinical and supervisory skills
  • Excellent organizational and interpersonal skills
  • Computer proficiency
  • Excellent verbal and written communication skills
  • Ability to read, write, and speak the English language
  • Maintain required vaccinations
