Job Title: Licensed Clinical Social Worker

We are seeking a highly skilled and compassionate Licensed Clinical Social Worker to join our team at Tenet Healthcare. As a Licensed Clinical Social Worker, you will play a vital role in providing high-quality patient care and support to our patients and their families.

Job Summary:

The Licensed Clinical Social Worker will be responsible for assessing patients' psycho-social needs, developing and implementing individualized care plans, and coordinating with healthcare teams to ensure seamless transitions of care. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Conduct comprehensive psycho-social assessments of patients to identify their needs and develop individualized care plans.
  • Collaborate with healthcare teams to ensure timely and effective transitions of care.
  • Provide education and support to patients and their families to promote healthy behaviors and improve health outcomes.
  • Develop and implement interventions to address patients' mental health and social needs.
  • Document patient information and progress in a timely and accurate manner.
Requirements:
  • Master's degree in Social Work (MSW) or related field.
  • Licensed Clinical Social Worker (LCSW) in the state of California.
  • At least 2 years of experience in a healthcare setting.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
