Job Summary

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to work with patients in their homes, conducting comprehensive health assessments and providing personalized care.

Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as necessary.
  • Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up care.
  • Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
Requirements
  • Masters degree required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certified by the AANP, ANCC, or the AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • Minimum of 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with employee health/medical record software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • Comfortable and flexible with frequent change.
Travel Requirements
  • Ability to travel, valid state driver's license, and ability to drive a car, proof of adequate automobile insurance coverage for the state of residence.
  • Travel required to meet patients where they live.
