Job Summary

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct comprehensive health assessments in the comfort of patients' homes, using our proprietary assessment tool to identify potential health risks and develop personalized care plans.

Key Responsibilities
  • Conduct adult/geriatric assessments, medical history, diagnosis, and treatment
  • Provide health education and collaborate with primary care physicians on patient care
  • Work collaboratively with case managers, social workers, and other healthcare professionals as needed
  • Travel to patients' homes, skilled facilities, and other community environments to provide care
Requirements
  • Master's degree in nursing or equivalent experience and licensure
  • Current RN and NP licensure in the state of practice, with prescription authority or ability to obtain
  • Board certification in a Matrix-approved specialty
  • 1+ year of experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
