Job Summary

The Medical Social Worker provides medical social work services to patients and families, assisting them with personal and environmental difficulties that may impact their health and well-being.

Responsibilities
  • Demonstrate a commitment to Northwestern Medicine's values and mission.
  • Provide accurate and timely information to patients and customers.
  • Address questions and concerns promptly, or identify appropriate resources to do so.
  • Use effective service recovery skills to resolve problems or service breakdowns.
  • Demonstrate teamwork by assisting colleagues within and across departments.
  • Communicate effectively with others, respecting diverse opinions and styles.
  • Use organizational and unit/department resources efficiently.
  • Manage work schedule efficiently, completing tasks and assignments on time.
  • Contribute to opportunities and processes for continuous improvement.
  • Participate in efforts to reduce costs and improve services.
  • Provide timely and direct medical social work services to patients referred by CNS staff.
  • Assess social and emotional factors limiting patients' medical treatment plan and assist patient/family with community resources as indicated.
  • Consult with physicians and health team members to understand significant psychosocial factors relating to patients' health and problems.
  • Provide counseling to patient/family for short-term and long-term planning and decision making.
Qualifications
  • Masters Degree in Social Work.
  • Two years of social work experience in the community home health area or hospital setting.
