AHN Healthcare@Home is seeking a dedicated and compassionate Registered Nurse to join our team of home health care professionals. As a Home Health RN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role

As a Home Health RN, you will be responsible for providing high-quality patient care in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by the physician
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Collaborate with the interdisciplinary team to ensure seamless care coordination
  • Communicate effectively with patients, families, and healthcare team members
Requirements

To be successful in this role, you will need:

  • RN license in the state of California
  • One year of RN experience and the clinical competence to deliver quality patient care
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones and EMR software
