RHA Health Services seeks a highly skilled Medical Case Manager Nurse to join our team. As a Medical Case Manager Nurse, you will be responsible for providing basic case management services to medically fragile children who are at risk of institutionalization.

Key Responsibilities:
  • Establish eligibility for services, obtain client histories, perform initial assessments, develop a plan to provide basic services, explain available services and programs, and refer clients to appropriate programs/agencies for other needed services.
  • Evaluate and document client programs.
  • Conduct interviews and work closely with clients, families, other relatives, school, medical providers, and other community agencies contacts to obtain information to be incorporated into a social history which will aid in planning for required services as appropriate to the individual plan of care.

The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Requirements:
  • Graduation from an accredited School of Nursing.
  • Ability to direct and carry out prescribed medical treatments and therapeutic programs.
  • Thorough knowledge of professional nursing theory and practice.
  • RN certification with one year of nursing experience preferred.
