Residential Reintegration Counselor

We are seeking a skilled and compassionate Residential Reintegration Counselor to join our team at Coordinated Care Services, Inc. The successful candidate will be responsible for providing case management services to individuals who have been accessed as needing addictions services.
Key Responsibilities:- Develop and implement individual client service plans, ensuring that plans are initiated and followed up as necessary.
- Assure that record-keeping is in compliance with Agency/OASAS regulations and policy & procedures.
- Provide counseling to residents to assist clients in the development of appropriate interpersonal, problem-solving, and decision-making skills.
- Respond appropriately to crisis/emergency situations to ensure/secure the safety/well-being of the clients.
- Responsible for the completion of discharge activities according to policies, practices, and procedures.
- Associates Degree in Health/Human Services
- Required CASAC-T within 30 days of hire
- Excellent written and verbal communication skills
- Demonstrated ability to communicate effectively and work cooperatively with culturally diverse persons, staff, and community service providers.
- Ability to multi-task, solution-driven & time management skills
- Ability to remain calm in a crisis
