Job Responsibilities- Provide individual and group counseling sessions to patients, utilizing evidence-based therapies and treatment modalities.
- Develop and implement individualized treatment plans, taking into account each patient's unique needs and goals.
- Conduct initial and follow-up assessments to evaluate patient progress and identify areas for improvement.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordinate referrals to other programs or facilities as needed.
- Provide crisis intervention and case management services to patients, ensuring timely and effective support.
- Act as a liaison between referral sources and patients, facilitating communication and ensuring seamless transitions.
- Bachelor's or Master's degree in a social or health services field from an accredited college or university.
- Prior experience and knowledge in an addiction recovery and/or medication-assisted treatment (MAT) environment.
- Licenses/Certifications: ADC, AADC, PLC, LPC, BSW, MSW (not required, but applicable to the position).
