Job Title: Clinical Pharmacist - Medication Management Specialist

Job Summary: We are seeking a Clinical Pharmacist to join our team as a PRN (as-needed) medication management specialist. The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care.

About the Role:

  • We are looking for a PRN Clinical Pharmacist who can take on a fluctuating schedule dictated by the needs of the business.
  • The successful candidate will be responsible for ensuring clinical accuracy and execution of patient prescriptions, verification of hard copy and digital Rx's, and supervision and execution of incoming and outgoing prescription transfers.
  • Additionally, they will perform DUR to identify MTM eligible patients and provide targeted interventions.
  • The Clinical Pharmacist will also ensure that all legal, privacy, and compliance requirements are met.

Responsibilities:

  • Deliver high-quality patient care through accurate and timely medication management.
  • Collaborate with healthcare professionals to ensure seamless care transitions.
  • Develop and implement individualized medication plans to optimize patient outcomes.
  • Communicate effectively with patients, families, and caregivers to educate them on medication use and potential side effects.
  • Maintain accurate records and reports to ensure regulatory compliance.

Requirements:

  • Bachelor's degree in Pharmacy or Doctorate in Pharmacy (Ph.D.)
  • Active Pharmacist license in good standing
  • Ability to work independently with minimal supervision
  • Strong analytical and problem-solving skills
  • Excellent communication and interpersonal skills
