Job Summary

Patriot Homecare is a rapidly growing, quality-focused, family-owned and operated local home health network seeking a skilled Physical Therapist Assistant to join our team.

As a Physical Therapist Assistant, you will provide skilled Physical Therapy services on an intermittent basis to patients in their homes under the direction of a Registered Physical Therapist.

Responsibilities
  • Under the direction of a Registered Physical Therapist, provide patient services which have been delegated by the PT.
  • Assist in evaluation and development of a rehabilitative plan of care and in periodic re-evaluation as required.
  • Participate in the preparation of clinical and daily progress notes.
  • Assume other duties deemed appropriate and necessary by the PT and as designated by the Registered Nurse Case Management and the Nursing Supervisor.
Requirements
  • Must be a graduate of an approved school giving a course of not less than two years for Physical Therapy Assistants.
  • Must be licensed by the State as a Physical Therapy Assistant.
  • Must have a criminal background check.
  • Must have current CPR certification.

This is a per diem position with a competitive rate.
