About the Position

The Nurse Case Manager coordinates all aspects of the patient's home care, interacting with other medical disciplines as needed and providing professional care to the patient. The admission visit to the patient in the home is performed to determine the patient's eligibility for home care services and develop the plan of care to be followed.

Responsibilities
  • Coordinate patient care services in the home
  • Develop and implement individualized care plans
  • Collaborate with healthcare teams to ensure seamless care delivery
  • Provide education and support to patients and families
  • Monitor patient progress and adjust care plans as needed
Requirements
  • Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing
  • Current state license as a Registered Nurse
  • Current state Driver's License
  • One year's experience as a Registered Nurse, two preferred
  • Proof of current CPR and Hepatitis consent/declination
  • Reliable transportation with valid and current auto liability insurance
