Social Services Case Manager

This role entails providing comprehensive social work services, including case management, to a diverse client population in compliance with state regulations and organizational policies.
Key Responsibilities:
- Conduct initial and ongoing assessments to determine client needs.
- Implement a variety of social work interventions to help families achieve their goals as outlined in the Service Plan.
- Deliver case management and face-to-face services tailored to meet client objectives.
- Maintain a flexible schedule to accommodate client requirements.
Qualifications:
A bachelor’s degree in social work (BSW) is typically required.
Experience:
A minimum of two years in a related field, such as child welfare or human services, is generally necessary. Proficiency in social work or case management is crucial.
Physical Requirements:
Ability to perform tasks such as sitting, standing, bending, stooping, reaching, and lifting items weighing up to 50 lbs. Adequate visual and auditory skills are also essential.
