Home Health Physical Therapist Assistant

We are seeking a skilled Physical Therapist Assistant to join our team at Pathwell Health. As a PTA, you will work directly under the supervision of a licensed Physical Therapist to provide ongoing therapy services to patients in their homes.
Key Responsibilities- Provide exceptional care to patients by delivering therapeutic exercises as planned, delegated, and supervised by the Physical Therapist.
- Communicate effectively with the patient's care team and follow instructions given by the assigned Physical Therapist.
- Perform ongoing therapy services, including but not limited to: application of heat and cold, massage, gait training, assistance and training in cardiopulmonary exercise, mobilization, assistance and training with ambulation, range of motion exercises, patient transfer, strengthening techniques, prosthetic training, and adaptive equipment.
- Accurately document care and patient progress in a timely manner, meeting established deadlines, communicating regularly with other caregivers, and documenting interdisciplinary communication.
- Confer with the licensed PT at least every 30 days to review the current patient care plan and make any necessary modifications to the treatment regime.
- Attend meetings, such as regular team meetings, in-services, and ad-hoc informational or educational meetings.
- Graduate of a Physical Therapy Assistant Program, accredited by the American Physical Therapy Association or equivalent program.
- Current and valid California Physical Therapist Assistant license.
- Recent work experience in home health is preferred.
- Driver's License, Car Insurance, and Car Registration.
- Reliable transportation and ability to drive in all types of weather conditions.
- Ability and willingness to follow agency policies and procedures and work as a team member.
- Strong interpersonal skills and good judgment.

The noise level in the work environment is usually low to moderate.
