MedStar Health is seeking a Social Worker (MSW) to become an integral part of our dedicated team.

The Social Worker plays a vital role within the interdisciplinary healthcare team, acting as a bridge between the hospital and the community.

This position emphasizes a psychosocial approach, essential for ensuring a seamless transition from acute hospital care to post-hospital services.


Key Responsibilities:
  • Provide social work and discharge planning services, including comprehensive assessment, strategic planning, and targeted intervention.
  • Identify patients in need of social work services through established case-finding methods.
  • Conduct biopsychosocial assessments involving patients and their families, adhering to departmental policies.
  • Offer counseling to patients and families regarding emotional adjustments related to illness, disability, hospitalization, and discharge plans.
  • Maintain effective communication with healthcare team members and community providers regarding patient status and discharge planning needs.

Coordination and Collaboration:
  • Facilitate patient care and departmental objectives by coordinating activities with hospital staff and referral agencies.
  • Demonstrate initiative and leadership in developing discharge care plans that meet patient care requirements.
  • Foster strong working relationships with healthcare team members, enhancing interdisciplinary patient goals through collaborative problem-solving.
  • Ensure effective communication between the hospital system and community agencies concerning patients' post-discharge needs.

Compliance and Quality Improvement:
  • Adhere to departmental standards regarding attendance, documentation, quality improvement, and compliance with the NASW Code of Ethics.
  • Document social work assessments, plans, interventions, and outcomes clearly in the medical record to facilitate communication with the medical team.
  • Continuously update knowledge of preferred providers, case management organizations, and insurance companies to enhance transition planning.
  • Identify and report opportunities for improvement and barriers to departmental and hospital effectiveness.
  • Engage in quality improvement projects and assist in the orientation and training of new staff.

Educational Contributions:
  • Provide educational experiences by supervising social work students and delivering presentations to staff and community groups.
  • Participate in hospital-sponsored training programs as required.
  • Contribute to inter-departmental teams and committees, sharing unit knowledge and demonstrating a constructive approach in all interactions.

Qualifications:
Bachelor's degree in social work with relevant experience is required. A Master's degree in social work is preferred.
