Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at DaVita. As a LPN, you will play a vital role in providing high-quality patient care and ensuring the comfort and well-being of our patients.

Job Summary:

The LPN will be responsible for providing dialysis-related nursing services in an outpatient setting. This includes performing patient assessments, providing chronic hemodialysis therapy, monitoring vital signs, collecting patient data, and assisting with machine and systems monitoring as required.

Responsibilities:
  • Provide patient-centered care to patients undergoing dialysis treatment
  • Conduct patient assessments and develop individualized care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient vital signs and report any changes or concerns to the healthcare team
  • Collaborate with the healthcare team to ensure seamless patient care
Requirements:
  • Current LPN-LVN license in the state of practice required
  • Current CPR certification required
  • IV certification required if required by state regulations
  • High school diploma or equivalent required
  • Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system
