Nurse Practitioner

• Assess and evaluate patient needs, developing individualized care plans to ensure optimal outcomes.
• Provide education and support to patients and families, promoting health and wellness.
• Collaborate with other healthcare professionals to coordinate care and achieve best patient outcomes.
• Document patient progress and updates, maintaining accurate and detailed records.

• Active Registered Nurse license.
• 1+ year of supervised clinical experience as a registered nurse, preferably in home health or med-surg.
• Excellent verbal and written communication skills, with strong interpersonal and time management skills.
• Strong computer skills and ability to navigate electronic medical records.
