Job Title: Registered Nurse

VNA Health Group is seeking a skilled Registered Nurse to join our team in providing high-quality home health care services to patients in Sayreville, New Jersey.

Job Summary:

The Registered Nurse will be responsible for assessing and evaluating the needs of patients with multiple disease processes and comorbidities, developing and implementing individualized care plans, and providing education and strategies for patients and families.

Responsibilities:
  • Assess and evaluate patient needs, develop and implement care plans, and provide education and strategies for patients and families.
  • Partner with other VNA clinicians and external providers to execute best patient outcomes.
  • Document all changes in patient condition and update care plans and outcomes.
Qualifications:
  • Active Registered Nurse license.
  • BSN a plus.
  • 1+ year of supervised clinical experience as a registered nurse, preferably in home health or medsurg.
  • Excellent verbal and written communication skills.
  • Solid interpersonal and time management skills.
  • Strong computer skills.
  • Strong problem-solving skills.
