Job Summary

We are seeking a skilled Speech Language Pathologist to join our team on a part-time basis. As a Speech Therapist with Residential Home Health and Hospice, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals.

Responsibilities
  • Develop and implement individualized patient care plans to address communication and swallowing disorders
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Provide evidence-based interventions and therapies to improve patient outcomes
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Master's degree in Speech and Hearing from a program certified by the American Speech and Hearing Association
  • Current licensure and certification as a Speech Therapist in the state of employment
  • Minimum of one year experience in healthcare, preferably in home health or hospice care
  • Ability to travel to patient locations, with a valid driver's license and reliable transportation
  • Strong communication and interpersonal skills, with the ability to work effectively in a team environment
