About the Job

We are seeking a skilled and compassionate individual to join our team as a Hospice On-Call Customer Service Representative. In this role, you will serve as a liaison between Hospice Home Care Planners, Social Workers, and RN Case Managers.


Responsibilities

This role requires strong communication skills to:

  • Respond to patient and customer complaints and grievances.
  • Enter patient referral information into the Allscripts system, including demographics, diagnosis, and plan of care.
  • Verify new physician license numbers, NPI registration, and check for duplicate entries.
  • Contact Physicians, Clinics, Hospitals, Nursing Homes, and sub-acute facilities to develop and follow up on pending referrals.

This is a full-time position with a schedule of 5pm to 1am.
