KEY RESPONSIBILITIES:
Delivers nursing care within the scope of training and expertise, providing direct care to residents.

Completes admission documentation and initiates care plans.

Administers medications and treatments in accordance with established protocols. Familiar with indications, actions, and potential side effects of administered medications.

Takes decisive action in urgent and emergency situations.

Proactively maintains a safe environment.

Implements individualized care plans for residents.

Supervises nursing assistant staff in direct resident care by:

Providing verbal and written reports at the start of each shift.


Observing CNA/resident interactions and the completion of resident care tasks including bathing, grooming, dressing, feeding, toileting, mobility, and participation in activities and appointments.

Discussing care plans for individual residents.


Conducting rounds and gathering feedback on resident status at the end of each shift or whenever there is a change in a resident's condition.

Communicating performance-related information on an ongoing basis.

Evaluates, in collaboration with the Unit Nurse/Supervisor/Unit Manager, the performance of CNAs.

Conducts frequent rounds on residents throughout each shift, observing for changes in condition and responses to care and treatment. Is knowledgeable about common health issues of older adults and usual treatments, incorporating that knowledge into daily practice.

Documents necessary and relevant information in the medical record.

Monitors residents with acute issues and communicates this to the Unit Manager or Supervisor.

Works to establish a therapeutic relationship with residents consistently, observing and upholding their rights.

Participates in patient care conferences to develop, assess, and revise care plans.

Engages in staff development programs and unit meetings.

Participates as assigned in quality assurance and performance improvement projects.

Channels administrative concerns appropriately to the Unit Manager or Supervisor, seeking guidance as needed.


Responsible for learning and utilizing standard functions within software and understanding basic computer operations to enhance efficiency in resident/patient service, assessment, and/or clinical documentation.

Responsible for using computers and software as required by the work environment to improve communication, performance, and knowledge.

Completes all assigned computer education modules promptly.

Takes responsibility for personal professional development


Is aware of Presbyterian Homes Corporate Compliance policies, including those related to confidentiality and information security, and the well-being of residents and patients.

Demonstrates behaviors and completes work in alignment with these policies.

EDUCATION:
Graduation from an accredited Practical Nurse program.

QUALIFICATIONS:
Active, current Illinois license to practice as an LPN.

Minimum of two years of experience as an LPN in a hospital (general care) or long-term care environment with adults. Strong communication skills. Interest in working with older adults. Ability to supervise nursing assistants. Knowledge of aging changes and common health issues of geriatric patients.

Current CPR certification.
