Job Summary

Sunrise Senior Living is seeking a skilled and compassionate Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) to join our team as a Wellness Nurse - Resident Services. In this role, you will be responsible for providing high-quality care and services to our residents, ensuring their overall well-being and quality of life.

Key Responsibilities
  • Resident Care and Services
    • Identify ongoing needs and services of residents to promote the highest quality resident services.
    • Ensure proper follow-through and participate in evaluation and documentation for residents with changes in healthcare services and status.
    • Assist the Resident Care Director (RCD) in conducting resident evaluations and gathering data at move-in, 30 days post-move-in, semi-annually, and with any change in status.
  • Wellness and Safety
    • Conduct monthly wellness visits for all residents.
    • Monitor and follow up with residents who have experienced falls, accidents, or incidents; document and report as required and directed.
  • Communication and Collaboration
    • Maintain communication with residents' families and responsible parties to ensure medical needs are being met.
    • Contact residents' attending physicians when necessary and/or upon family request.
  • Documentation and Record-Keeping
    • Document all pertinent information in the resident wellness file.
    • Maintain wellness files according to Sunrise policies and federal and state regulations.
  • Medical Support and Supplies
    • Ensure weights and vital signs are obtained monthly for each resident.
    • Maintain medical supplies and emergency kits for the community.
  • Quality Improvement and Infection Control
    • Understand and follow infection control practices.
    • Partner with the RCD and community team to promote infection control standards, including resident testing, training, and skills checks.
  • Medication Administration and Management
    • Regularly monitor each resident's medication and treatment profile to ensure each medication and treatment is administered as ordered and documented accurately.
Qualifications
  • Graduate of an approved college or school of nursing.
  • Maintains a current state license as a professional LPN/LVN.
  • Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrates knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
  • Demonstrates knowledge of good assessment skills.
  • Ability to handle multiple priorities.
  • Possesses written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care.
  • Competent in organizational, time management skills.
  • Demonstrates good judgment, problem-solving, and decision-making skills.
  • Demonstrates proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel), and Sunrise applications with the ability to learn new applications.
  • Ability to work weekends, evenings, and flexible hours, available to our customers at peak service delivery days and times.
