About the Role

At Abode Care Partners, we are seeking a dedicated Nurse Practitioner to join our team of healthcare professionals. As a Compassionate Healthcare Provider - Senior Care Specialist, you will play a vital role in delivering high-quality patient care to seniors and individuals with complex needs.

The successful candidate will possess strong interpersonal skills, excellent communication abilities, and a passion for providing evidence-based medical assessments and treatments.

Key Responsibilities
  • Providing Comprehensive Patient Care: Conduct thorough medical assessments, develop and implement comprehensive care plans, and ensure timely interventions to address acute and chronic conditions.
  • Collaborating with Healthcare Professionals: Work closely with caregivers, physicians, and other healthcare teams to provide seamless and interdisciplinary care.
  • Maintaining Accurate Medical Records: Ensure up-to-date patient records, incorporating relevant information from lab results, diagnostic testing, and treatment plans.
Requirements and Qualifications
  • Licensure and Certification: Active state Nurse Practitioners license, Current Nurse Practitioner National Board Certification, and Active state DEA licensure.
  • Education and Experience: Master's degree in Nursing (MSN) and at least five years of nursing experience, with two years of nurse practitioner experience preferred.
  • Skills and Abilities: Strong knowledge base of HIPAA and OSHA regulations, proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.

Salary Range: $100,000 - $120,000 per year.
