  • Communicate findings with patients' primary care physicians to inform care and address potential gaps
  • Provide education on disease processes, medication, and compliance to patients
Requirements:
  • Active NP or PA license or ability to obtain by start date
  • Active national NP or PA certification or ability to obtain national certification in state of assignment
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions
Preferred Qualifications:
  • 1+ years of clinical experience in a related setting
  • Experience in gerontology, cardiology, internal medicine, or endocrinology
  • Home health care or home visit experience
