Optum Home & Community Care is creating a new model of care that addresses an individual's physical, mental, and social needs. As a Clinical Assessment Specialist on our HouseCalls team, you will provide annual clinical assessments to patients in the comfort of their homes. This preventive care helps identify and reduce health risks for patients, coordinating follow-up care to improve health and well-being.

Key Responsibilities:

  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses to inform care management and active medical management.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Intervene in urgent and emergent situations.
  • Educate members on disease processes, medication, and compliance.

Requirements:

  • Active NP or PA license or ability to obtain by start date.
  • Licensure must be unencumbered, free of any open/unresolved disciplinary actions.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and dwelling conditions.

Preferred Qualifications:

  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
