Key Responsibilities:

- Collaborate with medical professionals to evaluate nutritional requirements, dietary restrictions, and existing health plans to formulate, implement, and document nutrition care strategies in PCC, while also offering nutritional counseling.
- Guide Residents and their families on dietary principles, intervention strategies, dietary adjustments, and food selection and preparation techniques.
- Oversee food service operations to ensure compliance with nutritional, safety, sanitation, and quality standards.
- Deliver nutrition services and medical nutrition therapy that align with the Residents' health objectives.
- Assess, interpret, monitor, and document the nutritional status and needs of Residents using established care guidelines.
- Educate individuals and groups on fundamental nutrition principles, healthy eating practices, and nutrition monitoring to enhance their quality of life while respecting cultural preferences.
- Issue diet orders as necessary.
- Investigate and address fluctuations in weight among Residents, discussing any concerns with nursing management to ensure proper follow-up and support.
- Participate as a member of the clinical interdisciplinary team.
- Maintain familiarity with individual care plans and contribute to the care planning process by attending meetings and making recommendations regarding Residents' nutritional needs.
- Accurately complete assigned MDS sections in a timely manner.
- Adhere to established safety protocols when performing tasks and utilizing equipment and supplies.
- Effectively engage with Residents exhibiting various cognitive statuses.
- Uphold the comfort, privacy, and dignity of Residents, interacting with them in a manner that conveys warmth, respect, and promotes a nurturing environment.
- Respond promptly and courteously to call lights when in Residents' care areas.
- Communicate and interact effectively and tactfully with Residents, visitors, families, peers, and supervisors.
- Maintain a high level of confidentiality in accordance with HIPAA regulations, ensuring that sensitive information is shared only on a need-to-know basis.
- Attend and engage in meetings and in-service training as scheduled, participating in educational programs required for maintaining professional certification or licensure.
- Understand and adhere to Infection Control protocols, including hand hygiene, isolation procedures, recognizing signs of infection, and managing infectious waste in compliance with OSHA standards.
- Promote and protect Residents' rights by assisting them in making informed choices, treating them with dignity and respect, safeguarding their personal belongings, reporting any suspected abuse or neglect, and supporting independent decision-making in accordance with applicable laws and regulations.
- Perform additional tasks as required.
This position does not entail supervisory responsibilities.
Qualifications:
- Must be a Registered Dietitian, or Registered Eligible, or hold a Bachelor’s degree in Dietetics.
- Completion of an accredited Dietetic internship program is preferred.
- One year of experience as a Registered Dietitian is preferred.
- Familiarity with LARA regulations is beneficial.
- Understanding of medical nutrition therapy in a healthcare or long-term care setting.
- Ability to communicate effectively, analyze challenges, and identify practical solutions.
- Must be able to relate well to older adults.
- Capability to organize and execute tasks with minimal supervision.
- Ability to collaborate effectively with colleagues within the department and facility.
- Strong interpersonal skills, with the ability to communicate effectively with families, volunteers, and the public.
- Flexibility to work varied hours, including evenings and weekends.
- Basic computer proficiency is required.
