Job Title: Physical Therapy Assistant Home Health

BAYADA Home Health Care is seeking a compassionate and skilled Physical Therapy Assistant to join our team in providing high-quality home health care services to our clients. As a Physical Therapy Assistant, you will work closely with our licensed Physical Therapists to develop and implement individualized treatment plans that promote optimal physical well-being and independence for our clients.

Responsibilities:
  • Assist clients in attaining or maintaining optimal physical well-being by providing treatment, tests, client education, and appropriate interventions as delegated by a licensed Physical Therapist.
  • Conduct home visits to assess clients' physical needs and develop treatment plans that address their specific goals and objectives.
  • Implement treatment plans under the supervision of a licensed Physical Therapist, including exercises, stretches, and other interventions as needed.
  • Provide education and support to clients and their families on proper use of equipment, exercises, and other interventions.
  • Document client progress and communicate with the interdisciplinary team to ensure seamless care coordination.
Requirements:
  • Current licensure or certification in the state of Vermont.
  • Graduation from a two-year college-level program approved by the American Physical Therapy Association, as indicated by school transcript or diploma, or two years of appropriate experience as a Physical Therapy Assistant and a satisfactory grade on a proficiency exam conducted, approved, or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977.
  • At least one year's work experience under the supervision of a qualified Physical Therapist.
