Responsibilities- Lead a team of RN Case Managers and Social Workers to ensure seamless patient care and efficient case management processes.
- Develop and implement effective case management strategies to reduce length of stay and avoidable days.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care and achieve optimal patient outcomes.
- Monitor and analyze case management metrics to identify areas for improvement and implement data-driven solutions.
- Provide guidance and support to team members to ensure they have the necessary resources and training to excel in their roles.
- Minimum 3 years of experience in case management, preferably in an inpatient hospital setting.
- Proven track record of success in reducing length of stay and avoidable days.
- Strong leadership and communication skills, with the ability to motivate and inspire team members.
- Ability to analyze data and implement evidence-based solutions.
- Current RN licensure and/or Social Work certification.
