About the Position

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct comprehensive health assessments, develop care plans, and provide education to patients and their families.

You will work collaboratively with our team of clinicians, case managers, and social workers to ensure seamless care coordination and optimal patient outcomes. Our flexible scheduling allows you to choose the days and hours that work best for you.

Responsibilities
  • Conduct comprehensive health assessments, including medical history, physical examination, and laboratory results
  • Develop and implement individualized care plans to address patient needs and goals
  • Provide education to patients and their families on disease management, medication adherence, and healthy lifestyle choices
  • Collaborate with our team to ensure seamless care coordination and optimal patient outcomes
  • Document patient information and care plans in our electronic health record system
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health record systems
  • Excellent verbal and written communication skills
