About the Role

We are seeking a skilled Community Health Worker to join our Care Management Support team at CenterWell Home Health. As a Community Health Worker, you will play a vital role in supporting our members in achieving optimal wellness by guiding them toward and facilitating interaction with appropriate resources for their care and wellbeing.

Key Responsibilities
  • Builds Visibility and Credibility: You will build visibility and credibility of Humana's products and services throughout the community via chats, posts, and interactions.
  • Engages New Community Members: You will engage new community members and strategize methods to scale communities and drive connections among all community members.
  • Conducts Non-Clinical Assessments: You will conduct in-person and telephonic non-clinical assessments to understand member care needs, preferences, and socioeconomic barriers, and evaluate the home environment as needed.
  • Collaborates with Care Teams: You will work collaboratively with other Humana associates as a member of our Care Management Support teams, including care managers and Wellness and Information Navigators.
  • Acts as an Enrollee Advocate: You will act as an enrollee advocate with providers, community resources, schools, and others, including accompanying member to provider office visits as requested.
Requirements
  • Residency in Oklahoma: You must reside in the state of Oklahoma.
  • Travel Requirements: Travel within the state is expected to meet with members - this role will primarily support members in Southeast Oklahoma.
  • Experience and Skills: You must have a minimum of two (2) years prior experience working with community resources, community health agencies/social service agencies (Area Agency on Aging, DME providers, Meal on Wheels etc.). You must also have exceptional communication and interpersonal skills with the ability to build rapport with internal and external customers and stakeholders.
Preferred Qualifications
  • Associate Degree: An associate degree is preferred.
  • Related Clinical Licensure or Certification: Related clinical licensure or certification is preferred.
  • Bilingual: Bilingual (Spanish, Native Tribal, Vietnamese, or other) is preferred.
