We are seeking a skilled Licensed Vocational Nurse to join our team at HIGH HOPE CARE CENTER. As a key member of our nursing staff, you will provide high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.
Key Responsibilities- Provide direct nursing care to residents, including administering medications, performing vital signs, and documenting resident information.
- Supervise and mentor Certified Nursing Assistants (CNAs) to ensure high-quality care and adherence to facility policies.
- Assess resident needs and develop individualized care plans to meet those needs.
- Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination.
- Participate in quality improvement initiatives and contribute to a culture of excellence in patient care.
- Current LVN licensure in the state of [insert state].
- Ability to work collaboratively as part of a team.
- Strong communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
