Key Responsibilities- Conduct thorough assessments of patients to identify their needs and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians and registered nurses.
- Monitor patients' progress and report any changes or concerns to registered nurses and physicians.
- Collaborate with registered nurses to develop and implement care plans that meet patients' needs.
- Provide education and support to patients and their families on health-related topics.
- Current CA LVN license and intravenous certification.
- BLS certification within 30 days of employment.
- Minimum 6 months experience in sub-acute care.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
